the Bondholder of an Issue-
Within the Summary
and Annual Reports the
reports are posted semi-annually. A
one
line summary of each account is listed with their paying agent. This
is the eighth column (Paying Agent) on
the report listing. Further detail on
the paying agent can be found on the Paying Agent address page located
earlier
in the report.
Change
my Contact Information-Within
the Forms button there is a .pdf document for the Account
Update Information. This
form
can be faxed, mailed or emailed back to our office.

Maximum
Reserve Requirements-On each
reserve Statements
of Accounts the maximum
requirement is noted. Please
select the range, then your period dates. The issues are listed alphabetical within each county.
The comment is
located at the bottom left of each statement with the notation
“RR:”. Once the reserve requirement is
met, any overage is surplus of the issuer and can be withdrawn at any
time. Most web browsers have a find/search feature.

have your board or town council approve the Resolution.
Complete
the bank information and account
listing forms. Mail all forms to the

need original
signatures, please no copies.
Sign
up for Autiomatic Sweep Payments-Please contact the Commission to obtain an applciation for the automatic sweep option. We will provide you with the necessary forms. Please
have your board or town council approve the Resolution.

Request
a Surplus Refund-Any overage
is surplus of the
issuer and can be withdrawn at any time. Request for surplus
refund must be on letterhead and with an authorized signature.
Fax letters are acceptable. Our fax number is
304-558-1280. Due to additional interest earnings, a sentence
is needed for review of future earnings to close an account.
